Web based PDF forms development project

We're looking for a web developer capable of creating web pages that allow for the collection of end-user data, insertion of the data into PDF forms that the end-user can print and then saving of that data and the pdf forms to a database. You should be familiar with PDF, LiveCycle and SQL databases.
